IDEA Transition Assessment, and Transition Planning

Is All about – Helping students with disabilities make informed choices and set realistic goals for their transition into adulthood.

We provide comprehensive evaluations using the latest industry tools and resources, to assess the special needs, skills and abilities, and set specific goals to:

•Enhance their chances of post-high school success,
•Improve skill development for post-school training and education,
•Increase their opportunities to enter the world of work, and
•Lead productive, integrated and independent lives within their communities.

Our Evaluations assist special  educators, students, and their families  to form a  realistic awareness of career options, and helps the individual  improve their self-awareness and motivation for Success.

The Plan has realistic short- and long-term goals for adult living that are individualized for each individual, and that include his or her particular capabilities and interests.

IDEA – Transition Assessment and Functional Vocational Evaluation:

Assessment is the first call to action when developing a transition plan.  Afull complement of both formal and informal inventories, assessments, and checklists of to identify  an individual’s current strengths, needs, interests, and preferences in the areas of~

•Career and vocation
•Life skills •Community participation
•Transportation
•Leisure•Post-secondary education
•Self determination

Consultation with parents of students with special needs:

Helping and involving parents of students with disabilities plan for their child’s future is a critical component of transition. And when families work closely with school systems, there is a better likelihood of success for their children in terms of post-secondary education or employment.

Consultation with special education departments:

Sometimes we are asked to design, develop and help implement IDEA/WOIA Vocational Transition Plans  for special education systems in public and private schools, and other ancillary agencies.

Transition planning:

With the reauthorization of IDEA 04 mandates and Indicator 13 measurements, transition planning has moved from a “nice concept” to a requirement. Ready or not, schools are being held accountable for service provision. Ms. Imperatore provides quality training and program design that assists students, parents and educators to plan for the future by~

•Understanding the law and mandates,
•Incorporating best practices in IEP planning and programming, and
•Providing transition services and supports.
